The Regional Extension Center (REC) in Massachusetts is ranked #2 in the nation for EHR incentive payments. In total, 2,487 primary care providers have used REC services and more than $7.5 million in Direct Assistance grants have been paid on behalf of MA providers.

Caring for the homeless population is among the most challenging jobs in the medical profession. The point of care can range from a health center to, sadly, a city street corner or park bench. Furthermore, many homeless need to worry about finding their next meal, staying safe and securing a place to sleep, before focusing on getting treatment for an illness or heading in for a regular check-up. The Boston Health Care for the Homeless Program is using ingenuity and innovation to help care for what is arguably the city's most vulnerable population. Doctors and other health care providers armed with tablets and laptops take to the streets to bring care directly to the patients. The electronic health record is a key component of keeping track of this largely transient population.

South Cove Community Health Center Uses Electronic Health Records to Increase Patient Follow Ups

South Cove’s work is a great example of what can be accomplished to improve health, health care and control costs when an EHR is used. In 2009, South Cove successfully transitioned entirely from a paper system to an exclusive electronic system using their EHR. South Cove uses its EHR to better track patient care by identifying those patients that are due for follow-up and wellness visits. It also closely tracks high-risk patients, which is often an arduous and time-consuming task.

Reliant Medical Group Pushes EHR to New Frontiers for Diabetes Care

When Reliant Medical Group implemented its EHR system a few years ago and began to using it track its diabetes patients, overall improvements in care outcomes for the disease were minimal. But when the medical group added a comprehensive alert system that reminds providers and patients about scheduled treatment, diabetes education and adherence to care plans, outcomes improved dramatically.
