Primary Care Provider (PCP)
Physician, such as a general practioner or internist, chosen by an individual to serve as his or her health-care professional and capable of handling a variety of health-related problems, of keeping a medical history and medical records on the individual, and of referring the person to specialists as needed.
Priority Primary Care Provider (PPCP)
MD, DO, Nurse Practitioner, Certified Nurse Midwife or dentist, who practices in the following settings:
- Primary care providers in individual and small practices (10 or less)
- Public and critical access hospitals
- FQCHC’s, community health centers and rural health clinics
- Other settings that serve uninsured, underinsured and medically underserved populations
Protected Health Information (PHI)
Any information relating to an individual’s medical records, health plan beneficiary information, physical or mental health information, or provided health services or any information collected during health service.
